Sorry for the bad drawing, but by the time it occurred to me to make a tutorial, I had already dismantled my rig and put the ladder back in the garage. But this will give you a basic idea.

hahahaha!!!! i thought i was MAD doing mine like this - great minds really MUST think alike.... ;) one thing i would add though - i have found it necessary to use a counter-weight hanging off the non-camera end of the tripod, or a very heavy weight on the top step part of the ladder to stop the whole rig toppling forwards and trashing my camera.
